Job Summary: - Asa Case Manager, you will deliver the successful operation of the CaseManagement function.
- Youwill act as a role model for care delivery and demonstrate WE CARE Valuesin all actions.
- Youwill demonstrate excellent service behavior.

Qualifications
- Graduate of an accredited school of Nursing -RNor MSW or Equivalent Masters prepared individual.
- Minimum of 2 years case management experiencepreferred
- Critical thinking skills, the ability to workwith minimal supervision, assertive and persuasive communication skills.
- Current GA RN License, BSN preferred or LicensedMSW or Equivalent Masters prepared
